Responsibilities

  • Providing direct care according to individualized care plans

  • Monitor client health status with

  • Medication reminders and treatments as directed by PT/OT.

  • Accurately document all visits and nursing interventions.

  • Communicate effectively with the SYNERGY HomeCare Admin Team and SYNERGY Registered Nurse.

  • Ensure compliance with state and federal home care regulations.

Qualifications

  • Current Minnesota Licensed Practical Nurse (LPN) license in good standing.

  • Current CPR certification.

  • Valid driver's license, reliable transportation, and current auto insurance.

  • Ability to travel throughout Mankato and surrounding communities.

  • Excellent communication and organizational skills.

  • Home care experience preferred but not required—we're willing to train the right candidate!

Position Overview The LPN Clinical Coordinator supports the delivery of quality patient care by balancing administrative responsibilities with occasional fieldwork.
