As a Licensed Vocational / Practical Nurse you’ll contribute to our success in the following ways: 

  • Follow each patient's individualized care plan and physician orders. 
  • Monitor patient progress and report changes or safety concerns to the care team. 
  • Help patients and caregivers learn self-care techniques to support their health. 
  • Work with the RN Case Manager to update care plans as patient needs change. 
  • Administer and document medications according to agency guidelines. 
  • Communicate with physicians, patients, families, and the care team to ensure coordinated care. 
  • Help prevent avoidable hospitalizations and emergency room visits through proactive patient care. 
  • Deliver compassionate, high-quality care that supports positive patient outcomes. 

What is Required?

  • Current State License as a Licensed Vocational / Practical Nurse 
  • Graduate of an accredited Certificate, Diploma, or Associate School of Nursing 
  • 1 year of experience in a clinical care setting 
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment 
  • Experience in a hospice or home health environment is preferred

Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician.
