JOB SUMMARY
The Admit Discharge Nurse is responsible for the admission interview, medication reconciliation, and discharge process. The position must possess excellent communication skills and function at a high level as a member of the collaborative team. The role of this position is pivotal in coordinating the care of the patient pre and post discharge to facilitate an efficient and effective acute care stay, and coordinate accurate information to the next provider(s) of care to minimize readmissions. This position is required to utilize independent judgment.
