Serenity Hospice PA is seeking a compassionate and dedicated Hospice RN Case Manager (RNCM) to join our growing team serving patients and families throughout Philadelphia, PA.

Your Role
As a Hospice RN Case Manager, you will play a key role in coordinating and providing exceptional end-of-life care. You will be the trusted clinical presence for your patients and families while collaborating closely with physicians, caregivers, and the hospice interdisciplinary team.
Responsibilities
Manage and coordinate individualized hospice patient care plans
Perform comprehensive nursing assessments and ongoing patient evaluations
Provide expert symptom management and compassionate nursing care
Educate patients and families regarding disease progression, medications, and comfort-focused care
Collaborate with physicians, caregivers, and interdisciplinary team members
Maintain accurate, timely, and thorough clinical documentation
Provide emotional support and guidance to patients and their loved ones
Advocate for patient comfort, dignity, and quality of life
What We’re Looking For
Active Pennsylvania RN license
Strong clinical assessment and organizational skills
Excellent communication and interpersonal abilities
A compassionate, patient-centered approach to care
Ability to work independently while being an engaged member of a team
Valid driver’s license and reliable transportation
