Abdominal Transplant Opportunities

Our Surgical Inpatient units are fast paced units that include high acuity patients. Are you looking to increase your knowledge base? Our Surgical Inpatient leadership team fosters an environment that supports learning, mentoring, and building each team member. 

 

-Patient Population: Liver, kidney, pancreas, auto islet cell, and uterus transplant patients, living liver and kidney donors, hepatology patients, hepatobiliary surgery patients, transplant preparation and more! Literature supports long term graft survival when care is provided by specially trained transplant RNs, so we see returning transplant patients who require admission regardless of diagnosis which allows us to build long-lasting relationships with this special population and their loved ones. 

-Patient Ratios: 1:4 on days, 1:5 on nights, 1:3 for certain step-down patients around the clock

-Number of Beds: 33

-Unit selling points: The program has performed over 8,000 transplants since its start in 1984 and is one of the oldest in the country. We were the first to successfully perform uterus transplant in the U.S. and performed over 450 organ transplants in 2021! We utilize a flexible self-scheduling process to support life-work balance with no set weekend rotation and staff scheduling committee for autonomy and collaboration. We also support team members in swapping shifts with each other if things come up last minute. We offer career growth opportunities within transplant starting with transplant specific certification (CCTN). JOB DESCRIPTION Opportunity for TX RN to join Molina as a Care Manager working with our Medicaid members in the northern part of the Fort Worth, TX service delivery area. Communities serviced will be North Richland Hill, Hurst, Richland Hills, and North Arlington. Responsibilities include completing in-home, face-to-face assessments with our waiver Medicaid members there. Preference will be given to RNs with this type of experience having worked for an MCO organization like Molina. Hours are Monday – Friday, 8 AM – 5 PM CST and mileage is reimbursed as part of our benefit package. Need to be bilingual. Solid experience with Microsoft Office Suite is necessary, especially with Outlook, Excel, and Teams as well as being confident in moving between different programs to complete the necessary forms and documentation. Job Summary Provides support for care management/care coordination long-term services and supports (LTSS)-specific activities. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Required Qualifications • At least 2 years of experience in health care, including at least 1 year experience in care management, managed care, and/or experience in a medical or behavioral health setting, and at least 1 year of experience working with persons with disabilities, chronic conditions, substance abuse disorders, and long-term services and supports (LTSS), or equivalent combination of relevant education and experience. • Registered Nurse (RN).
