A Day in the Life of a Mobile Integrated Health RN

The Mobile Integrated Health (MIH) Registered Nurse functions as an interdependent member of the care team who is responsible for performing comprehensive assessments, care coordination, medication inventory, patient and family education, diagnostic testing and medical treatments under the direction of a physician in a patient's home or current residence.

Here is What You Will Be Doing:

Conducts an in-home assessment of the patient which includes but is not limited to physical assessment, medication review and inventory, environment assessment and an evaluation of social support systems.
Conducts an extensive evalutation of the patient's medical and social history to develop recommendations that optimize care transitionsto the physician and other members of the care team.
Provides treatments and diagnostic testing in the home under the direction of a physician. Interventions and diagnostic tests include but are not limited to medication administration (route: intravenous, IM, SQ, oral, aerosolized), Point of Care Ultrasound device (POCUS), blood draws, speciman collections and ECG's.
Reads monitors and responds with correct intervention to ensure optimum patient care, safety and recovery.
Utilizes professional judgement, critical thinking skills and compassion to assist patients in overcoming barriers to goal achievement.
Leads daily interdisciplinary huddles which serve to prioritize patient needs determine essential treatments for patients scheduled for a visit that day.
Adjusts scheduled visits as needed to respond to urgent patient needs.
Promotes cross functional communication with multiple care teams such as ambulatory clinic, case management, home healthcare agencies, skilled nursing facilities, community organizations and health plan personnel by regularly communicating with care teams.
Engages patients through advocacy, ongoing communication, health education, identification of service resources and service facilitation.
Provides counseling and interventions related to treatment decisions.
Participates in process and quality improvement activities designed to improve workflow and patient outcomes. May include data collection, chart review, and participation in case reviews.
Initiates referrals for primary care, specialty care or community resources.

Associate's degree or higher from an accredited school of nursing
Wyoming Registered Nurse License or enhanced Nurse Licesure (eNLC)
Three (3) or more years of nursing experience
Valid USA driver's license
Must have clean driving record that follows Cheyenne Regional Medical Center's driving guidelines
14 Days: Cheyenne Regional AHA RQI within 14 calendar days of start date

Nice to Have:
