Essential Duties & Responsibilities:

  • Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
  • Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
  • Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
  • Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
  • Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
  • Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.

Travel Requirements This position supports members throughout Missouri (Primarily Eastern and Central). Approximately 75% travel is required. Overnight travel may be necessary based on assignment locations and member scheduling. Hotel accommodations, meals, and mileage reimbursement are provided for eligible business travel. Complete accurate, compliant, and timely EMR documentation. Follow organizational policies, clinical protocols, and regulatory requirements. Maintain professional licensure and certifications. Perform additional duties as assigned in support of organizational objectives. Required Active and unrestricted Nurse Practitioner (NP) in Missouri. National board certification applicable to NP specialty. Minimum of 3 years of direct patient care experience. Experience in primary care, family practice, internal medicine, geriatrics, urgent care, or related settings. Valid driver's license and reliable transportation. Ability and willingness to travel extensively throughout Southern Michigan. Preferred Experience with Medicare populations. Prior experience conducting in-home assessments or risk adjustment visits. EMR proficiency. Bilingual language skills. Familiarity with value-based care and quality gap closure initiatives. Physical Requirements Ability to travel frequently by own vehicle throughout assigned territories. Ability to enter and work within patient homes. Ability to lift and carry clinical equipment as needed. Ability to sit, stand, bend, and walk for extended periods during patient visits.
