In this role you will be responsible for the coordination of care on the nursing units in accordance with current applicable federal, state and local standards. This position will ensure that Senior Lifestyle standards for resident care are met.Job DescriptionResponsible for the proper maintenance of medical records to include monitoring documentation in Point Click Care. Monitor current health records, follow-up on incidents, issues with skilled residents and/or families, and meetings as needed.Develop and maintain a working relationship with professional providers of care to skilled residents. Assist the direct care nursing staff with admissions, transfers, and the discharge of residents.Monitor and follow-up with falls, infection control, skin issues, unusual occurrences, medication delivery error and any other issues as indicated by the DON. Maintenance of quality assurance parameters including chart audits, log audits and the scheduling of in-services as indicated.Oversee the daily completion and adherence to documentation as required in PCC.Complete audits as per direction of the DON.Assist the direct care staff with admissions, transfers & discharges.Provide direct care nursing and supervisor responsibilities as indicted.Provide staff in-service and education as indicated.Communicate, clarify and obtain orders, and updates on resident condition, etc. with other Health Care Providers.QualificationsLevel of Formal Education: Associate minimum, Bachelor’s Degree preferred Area of Study: Nursing Years of Experience: 5-10+ years Type of Experience: Direct resident care experience, previous experience in similar position, experience as a clinical nurse  Special Certifications: Must be RN/LPN Language Skills: Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with all others. Maintain compliance with all local, state, and federal regulations applicable to sheltered care. Resident Care Oversight Ensure that each resident’s health status is assessed upon admission and monitored regularly. Oversee the development and implementation of individualized service plans (ISPs) that meet residents’ personal and health care needs. Coordinate with residents’ physicians and other healthcare providers to ensure appropriate medical oversight and interventions as needed. Supervise medication administration and storage according to IDPH requirements. Staff Management Recruit, train, and supervise nursing and personal care staff, including Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and Personal Care Aides. Schedule and assign staff to ensure appropriate coverage for all shifts. Evaluate staff performance, provide coaching, and enforce standards of care and conduct. Ensure that nursing personnel are properly licensed and trained in emergency procedures, infection control, and resident rights. Quality Assurance & Compliance Conduct routine audits of resident records, medication administration, and care delivery. Monitor and report incidents, infections, and changes in resident condition to the Administrator and/or attending physician as required. Qualifications Required: Current Illinois Registered Nurse (RN) license in good standing. Minimum of two (2) years of experience in a long-term care, assisted living, or sheltered care setting. Demonstrated leadership and supervisory experience in healthcare. Strong knowledge of IDPH Sheltered Care Facility regulations (77 Ill. Adm. Code 330) and relevant long-term care standards. Preferred: Bachelor of Science in Nursing (BSN) or higher. Experience working with older adults and individuals requiring personal care rather than skilled nursing.
