Home health nursing gives you something that can be difficult to find in a traditional clinical setting: the time and independence to truly understand your patients.
As a Home Health Registered Nurse with Elara Caring, you’ll provide individualized care to patients throughout the Northampton territory. You’ll use your clinical judgment, manage your own caseload, and help patients recover, gain independence, and remain safely at home—all while being supported by a collaborative interdisciplinary team.

Your Role
Assess patients’ physical, psychosocial, environmental, and educational needs
Develop, implement, and update individualized plans of care
Manage an assigned caseload from admission through discharge
Provide skilled nursing care and patient education during home visits
Coordinate care with physicians, caregivers, therapists, and clinical leadership
Identify and communicate changes in condition or safety concerns
Complete accurate and timely clinical documentation
Advocate for each patient’s goals, independence, and quality of life
What You’ll Bring
Current, unrestricted Massachusetts RN license
At least one year of clinical nursing experience
Associate degree in Nursing required; bachelor’s degree preferred
Home health or hospice experience preferred
Homecare Homebase experience preferred
Dependable vehicle, valid driver’s license, and current auto insurance
Willingness to travel throughout the Northampton territory
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
