Everyday Care Advanced Practice Provider (NP or PA) - Responsive Care

Job Description

This role is 40 hours/week, fully remote and must be located within one of the contiguous 50 states. You must be able and willing to work Monday-Friday 8-hour shifts per week 8am-5pm MST/PST preferred, but willing to consider EST/CST working hours.

A bit about this role:

This position represents an amazing opportunity for an Advanced Practice Provider ( NP or PA) with a big heart and entrepreneurial spirit who is passionate about chronic disease management.

EDC, an integral part of Responsive Care, is the company's centralized hub for ongoing clinical management — a model built on a collaborative, shared patient management relationship with our members' primary care providers. In this role, you will work alongside a cross-functional team to deliver innovative care across Digital and traditional platforms.

You will serve as a member of our ONE PCP team, as a prescriber of centralized prescription renewals and refills and also you will manage centralized results.

You will utilize and help improve our home-grown technology and electronic health information platform to carry out virtual visits as well as managing asynchronous prescription refills for patients.

Required skills and experience:

  • APP with three or more years working in outpatient clinical practice ideally with experience in management of hypertension, hyperlipidemia and primary and secondary prevention of atherosclerotic cardiovascular disease.

  • Minimum of 2 years of experience concentrated in primary care or a subspecialty with heavy focus on hypertension and lipid management required (e.g. cardiology, nephrology, neurology, endocrinology, primary care, urgent care).

Licensure and certification

  • For Nurse Practitioners: active and in-good-standing RN and APRN licenses.

  • For Physician Assistants: an active and in-good-standing Physician Assistant license.

  • Role licensure and certification in good standing is required and the ability to get licensed in requested states within 90 days of hire date, starting at offer letter signing. You will be required to get licensed in additional states as needed. We believe in making it easy for you to focus on patient care. Devoted Health will cover all costs and provide full administrative support for obtaining and maintaining licenses in all required states.

Desired skills and experience:

  • Skilled APP with thorough understanding of cardiometabolic condition management including disease process, treatment modalities, medication management and lifestyle modification as it pertains to hypertension, hyperlipidemia, and primary care management of coronary artery disease and diabetes.

  • Comfortable working across Digital (live portal chat) and emerging AI-powered clinical tools (e.g., prescribing support and triage agents) as an active participant in testing, feedback, and iterative improvement — not just as an end user.

  • Virtual care experience is preferred along with a strong desire to continue practicing clinical nursing and performing virtual visits - you believe in the mission of bringing care to where the patient lives.

  • STARS/HEDIS measures, code clinical comorbidities, and identify clinical care gaps.

  • Experience with or strong interest in centralized results management, live digital/chat-based patient engagement, and AI-enabled clinical workflows is a plus.

  • Proficiency in English and Spanish preferred for this position.

Your Responsibilities and Impact will include:

  • Conduct focused and thorough assessments of patients with conditions that impact chronic health including hypertension, hyperlipidemia, coronary artery disease, and diabetes who are running out of medications. We expect that 40-60% of your clinical effort will be performing virtual visits.

  • Formulate accurate diagnoses and develop individualized treatment plans for patients with chronic medical conditions, including medication management, lifestyle modifications, and monitoring recommendations.

  • Manage a refill inbox for patients with chronic health conditions in accordance with established protocols and guidelines. We expect that 40-60% of your clinical effort will be spent managing a prescription refill queue.

  • Participate in centralized results management workflows, ensuring timely review, triage, and follow-up on lab and diagnostic results across the patient panel in accordance with established risk-tiering protocols.

  • Leverage Digital (live portal chat) and AI-assisted clinical tools — including agent-supported prescribing and inbox triage workflows — to improve efficiency, consistency, and safety in medication renewal, results management, and patient communication, while providing clinician feedback to help refine these tools as they evolve.

  • Identify and evaluate risk factors, comorbidities and possible contraindications for treatment.

  • Provide counseling on medication adherence, potential side effects.

  • Collaborate closely with a cross-functional care team — including PCPs, specialists, and other the company team members such as pharmacy, clinical nursing, and social work — as well as interfacing with family members and caregivers as needed to coordinate holistic care for the member, to ensure continuity of care and deliver a collaborative care plan.

  • Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface.

  • Maintain accurate and up-to-date patient medical records, ensuring compliance with relevant legal and ethical guidelines.

  • Participate in quality improvement initiatives and ongoing professional development to stay current on best practices and advancements in cardiometabolic care.

  • Adhere to all relevant laws, regulations, and industry standards, including patient privacy and telehealth regulations.

Salary Range: $120K-155K base range plus performance based bonus paid out quarterly or annually ($10K-$20K) for a total comp range of $130K-170K

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
