Virtual Urgent Care/Acute Care Advanced Practice Provider (NP or PA)

$120,000 - $155,000 / year
Job Description This role is fully remote and must be located within one of the contiguous 50 states. You must be able and willing to work 4-10 hour shifts per week, including one weekend shift per month, with working hours from 11am-9pm EST. A bit about this role: This position is an amazing opportunity for a caring Advanced Practice Provider (NP or PA) to help build and staff our growing telehealth medical group called Devoted Medical. Your primary focus will be delivering world class acute care to our members with emergent/critical illness. The Care OnDemand APP will diagnose complex medical conditions, order and interpret diagnostic tests, and work with patients, families, and Care OnDemand team to establish care plans. One of Devoted Medical’s missions is to bring care to where our members live meaning your visits will be virtual telehealth care . Required skills and experience: At least three years or more working as an APP, preferably in acute settings, urgent care, emergency medicine, and/or primary care or internal medicine with heavy acute management. Must be willing to work four-10 hour shifts per week, including one weekend shift per month, with working hours from 11am-9pm EST. Licensure and certification: For Nurse Practitioners: active and in-good-standing RN and APRN licenses. For Physician Assistants: an active and in-good-standing Physician Assistant license. Role licensure and certification in good standing is required and the ability to get licensed in requested states within 90 days of hire date, starting at offer letter signing. Experience in managing acute/chronic disease exacerbations including CHF exacerbations, diabetic emergencies, COPD exacerbations and hypertensive emergencies. Your Responsibilities and Impact will include: Performing Care OnDemand (acute care) visits including evaluating and diagnosing acute illnesses, ordering/interpreting diagnostic testing, establishing care plans including prescribing appropriate medications, and assessment for quality of care (STARS/HEDIS) interventions as well as social and home health/DME needs. Work closely with the member’s care team including their PCP, specialists, and other Devoted team members including pharmacy, clinical nursing, and social work as well as interfacing with family members and caregivers in order to coordinate care for the member and deliver a collaborative care plan. Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface. In certain geographies, there will be a weekend on-call component to support our clinical nurses who triage calls from our members during the weekend. As our healthcare environment and service needs continue to evolve, this role may expand to include additional responsibilities such as supporting new clinical/organizational programs/initiatives and integrating innovative virtual care practices. You must be able and willing to work Monday-Friday 8-hour shifts per week 8am-5pm MST/PST preferred, but willing to consider EST/CST working hours. Job Description This role is fully remote and must be located within one of the contiguous 50 states. This full-time position is designed to support weekend clinical coverage and requires regular Saturday and Sunday work, using a 3x12-hour shift. The 3x12s schedule will require some administrative and training time during an off-shift. These are mostly asynchronous and often can be done at your discretion. Job Description About the Role: Our Devoted Preventative Primary Care Provider roles, include Nurse Practitioners and Physician Assistants and will be considered for both roles. As a Devoted Preventative Primary Care Provider, you’ll be part of a mission-driven, interdisciplinary clinical team focused on improving the health, dignity, and quality of life of our members. You’ll meet patients in the first step of their primary care journey with Devoted Medical. You’ll conduct comprehensive clinical assessments, support chronic condition management, and emphasize preventative and proactive care. You’ll collaborate closely with primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families to deliver coordinated, member-centered care. This role sits at the intersection of preventative care, primary care and complex care management — it is not a traditional primary care panel role, and it’s not a one-time Annual Wellness Visit role. You’ll develop a relationship with your members while working in a supportive, tech-enabled environment designed to optimize clinical time and connection. What You’ll Do: Conduct primarily telehealth video visits, with limited and occasional in-home visits to members in your local area. Perform comprehensive assessments and provide evidence-based care focused on prevention, chronic condition support, medication optimization, and health maintenance. Identify and address care gaps, coordinate with external and internal care team members, and contribute to individualized, whole-person care plans. Utilize integrated technology and AI-enabled workflows to reduce administrative burden and maximize meaningful member interaction time. Communicate with empathy and clarity, fostering trust and confidence with members and caregivers. Required Skills & Experience: Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license. 3+ years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics. Active and clear NP/PA license (must be willing to obtain additional licenses if necessary within the first 90 days — support and reimbursement provided) Active BLS certification at time of hire. Comfort with delivering care via video telehealth and using electronic documentation systems. Preferred Experience: Familiarity with managed care models, including STARS/HEDIS and identification of care gaps. Experience performing comprehensive or preventive care visits with Medicare populations. Experience supporting older adults and/or individuals with complex medical or social needs.
