Polaris Healthcare and Rehabilitation Center, a leading provider of healthcare services in Milford, Delaware, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team! Responsibilities:

  • Provide direct patient care under the supervision of a Registered Nurse (RN) or other licensed healthcare professionals

  • Administer medications, treatments, and procedures as prescribed by healthcare providers

  • Monitor and record vital signs, medical conditions, and treatment outcomes

  • Assist with invasive and non-invasive procedures, including IV therapy and wound care

Requirements:
