Worcester Rehab is hiring an RN MDS Coordinator to join our reimbursement team!

Salaried role with weekly pay. Worcester Rehab is a 146-bed skilled nursing facility with a large SUDS population. $45 to $50 an hour, depending on experience.

You plan, organize, and direct the MDS process involving overseeing resident care plans through clinical assessment, review of resident's medical history, personal interviews, and completion of MDS reports.
Experience & Education:

  • Must possess, as a minimum, a Nursing Degree from an accredited school of nursing, college or university.

  • Must possess a current, unencumbered license to practice as an RN in this state.

  • Prior experience in MDS required.

Duties & Responsibilities:

  • Coordination of MDS process

  • Oversees ADL training for facility and staff.

  • Issues and delivers denial notices timely and appropriately.

  • Complete and transmit all CMS approved item sets (MDS)

  • Must be knowledgeable of and follow current CMS regulatory guidelines as described in RAI Manual.

  • Complies with facility privacy policies and procedures and protects residents’ individual health information.

  • Maintains Medicare meeting minutes per Medicare program agreement.

  • Issues and delivers Medicare denial letters per CMS regulations.

  • Assures appropriate management of residents’ Medicare/Insurance benefits.

  • Maintains adequate systems to ensure appropriate documents are sufficient to support billed services.

  • Other duties assigned by manager.

Specific Requirements:

  • Must be able to read, write, speak, and understand the English language.

  • Must possess the ability to make independent decisions when circumstances warrant such an action.

  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.

  • Must be able to coordinate MDS systems, resident assessment, and care plans for each resident timely.

  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.

  • Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.

  • Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents.

  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

  • Must be able to communicate effectively to appropriate personnel regarding emergency situations.

  • Must possess accurate and comprehensive assessment skills to ensure standards of nursing practice.

Provides patient/family teaching based on identified learning needs, readiness to learn, and barriers to learning. Collaboratively plans and prepares patient/family for discharge incorporating support systems/resources as necessary. Appropriately delegates and supervises nursing interventions performed by others. Appropriately prioritizes patient care (e.g. seeks assistance as needed from appropriate resources, reassigns personnel as necessary, recognizes need for emergency intervention, etc.). Provides thorough, timely, and accurate documentation of all pertinent data, therapeutic interventions and patient responses according to established standards. Provides timely and thorough report on assigned patients to other health care team members to ensure integration of services and continuity of patient care. Promotes learning for colleagues and self (e.g. functions as preceptor, instructor, trainer, attends professional development programs, etc.). Proactively identifies risk to patient safety and identifies opportunities to reduce medical/health care errors (i.e. appropriate reporting of errors/incidents). Identifies ethical issues in care and develops plan to address. Safely administers medications and intravenous therapy in accordance with physician’s orders, protocols, policies and procedures. Assesses patient pain level upon admission and as necessary, depending upon diagnosis utilizing the numerical pain scale. Reassesses effectiveness of pain control and notifies physicians as needed, as well as, documents assessment and patients’ response to medication. Responsible for obtaining supporting information and completing required documentation related to medication reconciliation for use by the Attending MD. Collaborates with surgeon, anesthesiologist, clinical coordinator and surgical technicians to assure the patients’ well being during surgical intervention. Maintains sterile fields and directs the correct care, handling, and sterilization of sterile supplies and instruments. Identifies patients correctly by checking patient identification bands and consent forms as well as verifying operative procedure and site before taking patients to the operating room. Applies principles of aseptic practice, oversees the selection and preparation of supplies, equipment and instrumentation to all surgical cases. Applies principles of body mechanics and adapts them to various situations during positioning and transfer of patients. Assures that all techniques and practices carried out during the surgical procedure are accurate and reflect standards of perioperative practice. Maintains sponge, needle and instrument counts according to policies and procedures, including notifying the surgeon of the result of the counts. Identifies, labels, cares for and handles all specimens and cultures according to established policy and procedure. Qualifications Qualifications : Current MA RN License required. ACLS upon hire OR must hold an active BLS card and obtain AHA ACLS within 60 days of hire. Recent OR experience required. CNOR preferred. Excellent communication and critical thinking skills required. Must take call per unit rotation.
