Job Summary:

Winchester Rehabilitation and Nursing Center, a 120-bed skilled nursing facility, is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled nursing team. As an LPN at our facility, you will provide direct nursing care, supervise nursing support staff, and help ensure high-quality, resident-centered care in accordance with facility policies, clinical standards, and regulatory requirements.

Responsibilities:

• Supervise Certified Nursing Assistants (CNAs) and other nursing personnel on assigned units.
• Provide direct nursing care, including medication administration, wound care, colostomy changes, tube feedings, suctioning, IV administration, and other skilled nursing interventions as assigned.
• Observe, document, and report changes in residents' conditions to physicians, families, and nursing leadership.
• Accurately transcribe and carry out physician orders.
• Participate in resident admissions, discharges, and transfers.
• Conduct regular rounds to ensure resident needs are being met and staff are following facility policies and procedures.
• Schedule follow-up appointments and coordinate transportation needs for residents.
• Collaborate with the interdisciplinary care team to support comprehensive resident care.
• Maintain adequate supplies of medications, equipment, and other necessary resources and notify appropriate personnel when additional resources are needed.
• Promote teamwork, mutual respect, and effective communication among staff.
• Participate in regulatory surveys, inspections, and Quality Assurance and Performance Improvement (QAPI) activities.
• Treat all residents with dignity and respect while protecting resident rights and maintaining confidentiality.
• Follow infection control policies, safety procedures, and life safety and emergency protocols.
• Report abuse, neglect, exposure incidents, or noncompliance in accordance with facility policies and applicable regulations.
• Complete all required training and skills competency requirements.

Qualifications:

• Current, unencumbered Massachusetts LPN license.
• Strong communication and critical-thinking skills.
• Ability to handle challenging situations with tact and professionalism.
• Ability to work effectively with residents, families, and members of the interdisciplinary care team.
• Prior long-term care experience preferred but not required.
• Compassionate, dependable, and committed to providing high-quality resident care.
