Responsibilities:

  • Provide direct patient care, including administration of medications, wound care, and vital sign monitoring

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Demonstrate excellent communication and interpersonal skills to ensure effective communication with residents, families, and healthcare professionals

  • Participate in quality improvement initiatives and maintain accurate and up-to-date medical records

  • Collaborate with the nursing team to ensure continuity of care and optimal outcomes

  • Perform other duties as assigned by the Registered Nurse Manager or Director of Nursing
