Job Responsibilities

Delivering nursing care in post-acute facilities
Following resident care plans with consistency and accountability
Administering medications, verifying dosages, and monitoring their effects on residents
Monitoring the physical and mental wellbeing of residents, including tracking vital signs
Participating in IV setup, immunizations, and phlebotomy procedures
Reviewing and updating residents' charts and medical records as needed
Conducting resident intakes and accurately documenting health concerns and medical histories
Helping to foster a safe and comfortable environment within the facility

Qualifications and Skills

Applicable state-issued LPN license
Successful completion of approved LPN program
Minimum of 6 months of experience (LPN or comparable role)
Ability to lift a minimum of 50 lbs
