Job Summary:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Plaza Healthcare in Elizabeth, NJ. As an LPN on our weekends team, you will play a crucial role in providing high-quality patient care and supporting our healthcare team. Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital sign checks
• Develop and implement individualized care plans in collaboration with the healthcare team
• Monitor and report any changes in patient condition to the nursing staff and physicians
• Assist with daily living activities, such as bathing, dressing, and grooming
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction
• Collaborate with the interdisciplinary team to ensure comprehensive care
