The LPN or RN will provide direct nursing care to the residents and will perform the day-to-day nursing activities in accordance with current federal, state and local standards governing the facility, and as directed by the Director of Nursing Services, Assistant Director of Nursing Services and Administrator, to ensure that the highest degree of quality care is maintained at all times.

Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing and meet all applicable federal and state licensure requirements.
Must speak and understand English. - Must be knowledgeable of nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care.
Positive attitude toward the elderly.

Overview Support Families & Patients in home setting 8:00AM – 8:00PM or 8:00PM – 8:00AM We are seeking a dedicated Continuous Care Licensed Practical Nurse (LPN) to join our team. Reporting to the Executive Director/Administrator or Director of Clinical Services, and working under the direction of a Registered Nurse, you will provide continuous nursing care to hospice patients in facility or home settings. Your role is vital in monitoring patient conditions, managing acute symptoms, and supporting families during critical times. Key Responsibilities Provide skilled bedside nursing care following the patient’s plan of care and under RN supervision. Monitor patients actively, report changes in condition, and manage acute medical symptoms to achieve palliation during crisis periods. Administer medications, monitor vital signs, and evaluate patient and family needs. Assist with plan of care preparation and execution, including infection control and symptom management. Document patient care activities and communicate findings with interdisciplinary teams and physicians. Participate in bereavement support and may assist with death pronouncements as allowed by state law. Provide guidance to hospice aides under RN supervision as per Nurse Practice Act regulations.
