Home Health RN (PRN/1099) – Metro Atlanta

Gaddiel Home Care Services, Inc. is seeking Registered Nurses (RNs) to join our team as in-home assessment specialists. As a 1099 contractor, your primary responsibility will be conducting 60–90 day client assessments in the comfort of their homes throughout the Metro Atlanta area.

Key Responsibilities:

  • Conduct comprehensive 60–90 day in-home client assessments

  • Evaluate patient health status, needs, and care plans

  • Communicate assessment findings to the care team

  • Provide skilled nursing care as needed, including:

  • Medication management

  • Wound care

  • Catheter and tube feeding care

  • Diabetes management/insulin administration

  • Colostomy, ileostomy, and urostomy care

  • IV therapy and other skilled nursing duties

Qualifications:

  • Current and valid RN license in the state of Georgia

  • Minimum 1 year nursing experience; home care experience preferred

  • Strong assessment and clinical skills

  • Excellent communication and interpersonal abilities

  • Ability to work independently with clients in their homes

  • Reliable transportation for travel throughout Metro Atlanta

We are seeking a Field Nurse Case Manager to cover Atlanta to include but not limited to Athens, Gainesville, Lawrenceville, Douglasville, McDonough. A Field Nurse Case Manager is a hybrid position. The candidate must be located in the Atlanta area due to regular local travel (80% of the time) for in-person patient appointments. Candidate must be willing to travel and be licensed in GA. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers.
