Looking for an RN opportunity that combines clinical nursing with independence and community-based care? At Home Care Partners is hiring a Home Care Field Registered Nurse to serve clients throughout Hamilton County, NY.

You'll conduct patient assessments in home and community settings, develop individualized Plans of Care, supervise aides, review medications and diagnoses, monitor changes in condition, and coordinate with families, caregivers, and our internal team.

Candidates must have an active New York State RN license, Medical Professional Liability insurance, reliable ability to travel throughout the assigned service area, strong documentation and communication skills, and basic proficiency with Gmail and Microsoft Word/Excel.

Previous home care, home health, outpatient, or community nursing experience is preferred. Knowledge of Medicaid, Medicare, or MLTC plans and experience developing Plans of Care are also preferred.

Compensation: $85,000 per year
