At Home Care Partners is seeking a compassionate and experienced Home Care Field Registered Nurse (RN) to support patients throughout Madison County, New York.

If you're an RN who enjoys building relationships with patients, working independently, and providing personalized care outside of a traditional facility setting, this position offers the opportunity to make a direct impact in the community.

As a Field RN, you will visit patients in their homes and community-based settings to complete clinical assessments, oversee Plans of Care, support home health aides, and help ensure every client receives safe, appropriate, and individualized care.

What You'll Do

  • Travel to patient homes and community-based settings throughout Madison County

  • Conduct comprehensive in-home nursing assessments

  • Perform head-to-toe evaluations and review medications and diagnoses

  • Develop, review, and update individualized Plans of Care (POC)

  • Complete required aide supervisions

  • Provide clinical guidance and support to home health aides and caregivers

  • Orient aides to each client's Plan of Care and specific care needs

  • Monitor patients for changes in condition and communicate concerns appropriately

  • Collaborate with patients, families, caregivers, aides, and office staff

  • Complete accurate and timely clinical documentation

  • Help ensure services are delivered according to each client's individual needs

  • Maintain compliance with applicable healthcare regulations and company standards

What We're Looking For

  • Current and active New York State Registered Nurse (RN) license

  • Ability and willingness to travel throughout Madison County and assigned service areas

  • Medical Professional Liability insurance

  • Strong clinical assessment and critical-thinking skills

  • Excellent written and verbal communication skills

  • Strong documentation and organizational abilities

  • Comfortable using Gmail and Microsoft Word/Excel

  • Ability to manage responsibilities independently in a field-based environment

Preferred Experience

  • Home care or home health nursing

  • Community health or outpatient nursing

  • Conducting in-home patient assessments

  • Familiarity with Medicaid, Medicare, or MLTC plans

  • Bilingual Spanish/English is a plus

Implement and carry out a medically based group for patients on the above topics. Serves on special review and quality assurance committees as assigned. Responsibilities Work under the supervision of the Nurse Manager, Service Director and/or Program Director and follows medical orders for patient care. Communicate with supervisor, medical provider and/or Medical Director regarding patients' medical concerns. Monitor patients' medical status, signs and symptoms of withdrawal and reports to supervisor and/or medical director. Work with other medical staff across the agency and in the community for continuity of patient medical care. Communicate with clinical staff regarding patients' medical concerns. Work directly with patients' in providing medical services, interpreting medical information and provide recommendations or make referrals for medical services. Develop, recommend, and review medical policies and procedures for all programs within Syracuse Behavioral Healthcare. Advise the agency and ensure the provision of services for medical and health related issues. Provide direct patient care of medical examination, implementation of medical orders, treatment of medical conditions and communicable and infectious disease reporting in accordance with the law. Execute and review public health education and screening protocols with regards to tuberculosis, sexually transmitted diseases, hepatitis, and HIV prevention and harm reduction in accordance with Article 27-F of the Public Health law and under the Mental Hygiene law in coordination with the Medical Director/Nurse Manager. Implement and carry out a medically based group for patients on the above topics. Serves on special review and quality assurance committees as assigned. Provide and document medical consultation, assessment, examination, and treatment planning and review consistent with agency policy and requirements. Other related assignments and duties consistent with training and experience. Qualifications Must be a licensed Registered Nurse currently registered by the New York State Department. At least one year of experience, or comparable training in the treatment of substance abuse and/or substance dependency. Basic computer skills required.
