We are looking for a dedicated, caring Licensed Practical Nurse to join our team. What You'll Do

Administer oral, topical, and injectable medications per physician orders
Monitor residents for changes in condition and report to the supervising RN
Perform wound care, catheter care, ostomy care, and treatments
Obtain and document vital signs, weights, and intake and output
Implement care plans as directed by the RN and care team
Supervise and delegate tasks to CNAs on your shift
Communicate with residents, families, and the nursing team
Document care accurately and timely in the electronic health record
Follow infection control, safety, and facility compliance standards

Requirements

Current, unrestricted LPN license in the state of practice
Graduate of an accredited practical nursing program
Current BLS/CPR certification
Long-term care or geriatric nursing experience preferred
Strong medication administration and observation skills
Ability to work rotating shifts, weekends, and holidays

A Brief Overview Children's Hospital of Philadelphia is seeking a Licensed Practical Nurse for their Chadds Ford Primary Care office. This position will be Monday-Friday and will require one evening shift per week and a rotating Saturday shift. The shift will be based off the operational need. Two years of LPN and pediatric experience is preferred. The Licensed Practical Nurse performs routine nursing care of the ill, injured or infirm under the direction of a licensed professional (RN) nurse, nurse practitioner and/or a physician. The LPN participates in the planning, implementation and evaluation of nursing care in accordance with the scope of the LPN as defined by the State Board of Nursing and health system standards. Also supports operations through administrative duties as required. The nurse must demonstrate competence in caring for patients with moderate to complex health issues and assist the Site Manager with administrative issues which affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols as outlined in policy and procedure manual. Administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues. Must be willing to rotate if needed to other Care Network sites within the designated cluster. What you will do Provide and document direct nursing care of assigned patients under the supervision of a registered nurse or physician. Nursing care is guided by physician orders and the plan of care. Patient response to care is reported to a registered nurse or physician for evaluation, intervention and modification of the plan of care.
