This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state of Ohio Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and aide leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own on a day-to-day basis with activities which include: phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, and meetings (i.e.: schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. Job Summary The on-call RN is responsible for coordinating care, providing direct care/support for hospice patients and their families in the home and various nursing facility settings. The on-call nursing schedule runs Saturday thru Monday from 8pm - 8am, and Mondays 5 pm-8am. Flexible schedule, full time, part-time per diem. YOU DO NOT CARRY PHONES-triage nurse handles calls, you only go out if needed. Responsibilities and Duties On-call support after agency hours for hospice patients/families with regard to comfort, symptom management, medication management, DME needs, admissions, discharges, proper documentation and coordination of care between hospice interdisciplinary team & facilities.
