As a Licensed Practical Nurse (LPN) with our team, you will play a vital role in delivering high-quality patient care while working collaboratively with our interdisciplinary team.

About the Role:

As a Licensed Practical Nurse (LPN) with Emerald Hills, your primary responsibilities will include:

  • Providing direct patient care, including administering medications, performing wound care, and assisting with activities of daily living

  • Developing and implementing individualized care plans in collaboration with the healthcare team

  • Monitoring patient progress and reporting changes to the nursing staff and medical director

  • Maintaining accurate and detailed records of patient care and treatment

  • Collaborating with the healthcare team to ensure seamless transitions and referrals

  • Participating in quality improvement initiatives and professional development opportunities

Ideal Candidate:

  • Current LPN license in the state of New Jersey

  • 1+ year of experience in a long-term care or rehabilitation setting

  • Strong communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment with multiple priorities

  • Compassionate and patient-centered approach to care
