What You’ll Do

Provide high-quality nursing care to residents in accordance with individualized care plans.
Administer medications and treatments as ordered.
Monitor residents’ conditions and report changes to the RN and medical team.
Complete accurate and timely clinical documentation.
Work closely with CNAs, RNs, physicians, and other members of the interdisciplinary team.
Assist with assessments, care planning, and resident needs.
Promote resident safety, dignity, comfort, and quality of life.
Communicate effectively with residents and their families.

What We’re Looking For

Current New York State LPN license.
Previous nursing experience preferred; skilled nursing experience is a plus.
Strong clinical, communication, and organizational skills.
Compassionate and resident-focused approach.
Ability to work effectively as part of a team.
