As a Hospice LPN You will:  

Give hands-on care to patients nearing the end of life, following state nursing rules.

Follow the care plan after assessing the patient’s needs.

Keep clear and timely records of the care provided.

Teach and support patients and their families about the illness and care plan.

Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).

Manage pain and symptoms, checking how well treatments are working.
Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications

Graduate of an accredited practical nurse or vocational nursing program.

Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.

Currently licensed as an LPN/LVN in the State.

Comply with accepted professional standards and practice.

Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Possess and maintain current CPR certification.

Understand philosophy of hospice concept. Also understand needs of the terminally ill.

Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.

Demonstrate good verbal and written communication, and organization skills.

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences. Serenity Hospice Care is looking for compassionate and dedicated Licensed Practical Nurses to join our team in New Jersey. This individual will be responsible for care & services to patients as needed. Candidates must hold a valid LPN License in the state of New Jersey in order to be considered. This position will be 7PM to 8AM and will be 1:1 for patients. Responsibilities: Care for patients in the location as needed. Assist with activities of daily living. Follow care plans and deliver the highest level of care. Document medical information as required. Collaborate with interdisciplinary team. Communicate with families and patients.
