Elderwood at Hornell is seeking an experienced Registered Nurse Unit Manager to provide clinical and operational leadership for one of our nursing units.

 

This is a visible, hands-on leadership position for an RN who can bring structure, accountability, sound clinical judgment, and consistent support to a nursing team. You will partner closely with the Director of Nursing, Assistant Director of Nursing, providers, rehabilitation professionals, and other members of the interdisciplinary team to strengthen nursing practice and resident outcomes.

 

You will have meaningful accountability for your unit - but you will not be expected to lead without support. Elderwood's facility leaders are backed by experienced onsite leadership and the broader clinical resources of Elderwood Administrative Services.

Lead With Support Behind You

As RN Unit Manager, you will provide 24-hour oversight and accountability for your assigned unit while working primarily on a generally day-shift schedule with flexibility based on facility and unit needs.

 

Facility clinical teams receive support from Elderwood Administrative Services, including organization-wide clinical leadership from the Chief Nursing Officer and an assigned Regional Nurse Consultant who serves as a resource to facility leaders.The Regional Nurse Consultant partners with the DON, ADON, Administrator, and clinical team to provide guidance, share best practices, help navigate complex challenges, and support priorities related to quality, regulatory readiness, clinical operations, and nursing leadership development.

Responsibilities

Unit Manager (RN) - Skilled Nursing - What You’ll Lead:

Unit Leadership & Team Oversight

  • Provide 24-hour oversight of your assigned nursing unit
  • Supervise, coach, and develop nursing staff — with a strong focus on CNA support and performance
  • Set clear expectations for care delivery, accountability, and teamwork

Clinical Oversight & Resident Care

  • Oversee development, implementation, and evaluation of resident care plans
  • Monitor clinical indicators and proactively address resident needs
  • Ensure high standards in documentation, infection control, and regulatory compliance

Family & Physician Communication

  • Serve as a key point of contact for families and providers
  • Communicate changes in condition and care plans clearly and professionally
  • Address concerns and support service recovery when needed

Operations & Staffing

  • Manage daily staffing, call-ins, and scheduling needs
  • Participate in hiring, performance management, and staff development
  • Conduct daily rounds to ensure quality and environment standards are met

Qualifications

Unit Manager (RN) - Skilled Nursing - What We’re Looking For:

  • Active Registered Nurse (RN) license - New York State
  • Minimum 2+ years of nursing experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting
  • Prior leadership experience (Charge Nurse, Unit Manager, Supervisor) required
  • Strong ability to lead, coach, and hold staff accountable
  • Experience working with CNA teams and developing newer staff
  • Confidence in handling family communication, clinical oversight, and regulatory expectations
  • Experience with dementia population strongly preferred

Infection Prevention/Educator RN - Skilled Nursing Facility

The SNF Infection Prevention/Educator RN is a professional whose knowledge of nursing procedures, aseptic techniques and good communication skills assists in the ability to educate physicians, nurse and other Arnot Health personnel in the prevention and transmission of infections. This position is an assistant to the Infection Prevention System Director and requires working closely with the daily implementation, surveillance, reporting of infections, evaluation of isolation guidelines and policies and participation in performance improvement and best practice compliance. Reports to the SNF Director of Nursing. Duties And Responsibilities Is the leader of the SNF infection prevention and control program (IPCP). Formulates educational/staff development programs utilizing total age appropriate criteria which meets the needs of individual patients. Utilizes theories of teaching and learning to meet identified patient/family and staff educational needs. Coordinates the orientation process of new employees and serves as a resource for preceptors. Collaborates with the Director of Nursing to identify educational needs within the unit/department, and formulates a written goal-directed plan of education to address priority areas. Is responsible for providing staff development, educational programs, and inservicing, as appropriate, to cover all staff/all shifts. Maintains appropriate documentation of orientation activities and on-going educational programs including objectives, class content, evaluations, and employee attendance. Investigates and reports suspected outbreaks to DOH by completing NORA reports.
