  • Holds an active Licensed Practical Nurse license, and is eligible to practice in the state of Kentucky. 
  • Currently certified in CPR.

Minimum Work Experience

  • Two years experience as a licensed practical nurse.
  • Home Health scheduling experience preferred.

Essential Functions

  • Provides direct nursing service as ordered by physician including specialized care to pediatric, adult, and geriatric patients, case management as indicated.
  • Documents the delivery of patient care.
  • Provides Case Management for Home and Community Based Service.
  • Educate patient and/or caregiver on disease management and consults with the physician as needed.
  • Reviews, educates and updates patient medications accurately and timely.
