Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

  • Must be a graduate of an approved school of clinical education.
  • Must be licensed in the state where they currently practice.
  • Must have at least one year experience as a licensed professional.
  • Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

  • Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
  • Medicare home health or hospice experience is preferred.

Requirements

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider

The Registered Nurse plans, coordinates, and provides patient care to include interdisciplinary planning, discharge planning, patient and family teaching for post-hospitalization, and accessing community resources. May perform other related duties as assigned or requested, including Charge Nurse (ACLS certification required). Qualifications Education and Training: Holds a current licensure as a registered nurse (RN) in the state where the hospital resides, or compact licensure if state participates. Current BLS certification is required. Evidence of ACLS certification is required within 90 days of employment. BLS and ACLS certifications must be AHA approved within 90 days of employment. Current ACLS certification is required for Charge Nurses. Experience: One year of inpatient medical-surgical nursing experience preferred but not required.
