Job Description

Facilites clinical admissions to inpatient acute/sub-acute rehabilitation care. Serves as a liaison for referral sources and promotes available rehabilitation services. Communicates required information to the inpatient clinical team.

Job Responsibility

  • Receives referrals for rehabilitation care from internal/external inpatient/outpatient facilities.
  • Performs clinical screening for admission based on regulatory, department and insurance guidelines. Reviews necessary medical records and may visit referring facilities of patients to be admitted.
  • Identifies, communicates and coordinates for any special needs of patients during admissions process.
  • Collaborates with required stakeholders (ie. physicians, clinical professionals, administrative and clinical support team members) to facilitate admissions process.
  • Completes and processes required admissions documentation.
  • Establishes and develops community/physician referrals for possible admissions.

Job Qualification

Graduate from an accredited clinical eduation proram, required.
