A skilled nursing facility in the Lima, Ohio area is seeking an experienced MDS Coordinator to join our clinical leadership team. This is a great opportunity for a detail-oriented nurse who understands the importance of accurate assessments, strong documentation, regulatory compliance, and maximizing quality outcomes.
Responsibilities
Complete and coordinate MDS assessments accurately and within required timeframes
Manage the assessment schedule, including ARDs and required submissions
Ensure accurate coding and supporting clinical documentation
Coordinate with nursing, therapy, dietary, social services, and other interdisciplinary team members
Support care planning and Care Area Assessment (CAA) processes
Monitor documentation for accuracy, completeness, and regulatory compliance
Maintain knowledge of PDPM, Medicare requirements, and current MDS guidelines
Participate in audits, quality initiatives, and survey preparation
Educate and support facility staff regarding MDS-related documentation requirements
Requirements
Active Ohio RN or LPN license
Previous MDS experience in a skilled nursing/long-term care setting required
Strong knowledge of MDS 3.0, PDPM, Medicare, and reimbursement
Experience with PointClickCare (PCC) preferred
RAC-CT certification preferred, but not required
Strong Microsoft Office skills, including proficiency with Excel, Word, and Outlook
Strong organizational skills and attention to detail
Ability to collaborate effectively with an interdisciplinary team
