Our experienced clinical team is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities.

About the Role:

We are seeking a highly skilled and dedicated NP or PA to join our team of healthcare professionals. As a hybrid role, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Our ideal candidate will have a strong background in Geriatrics or Internal Medicine and excellent communication, collaboration, and teamwork skills.

Responsibilities:

  • Deliver high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work

  • Conduct patient assessments and develop treatment plans in collaboration with the healthcare team

  • Provide chronic care management and remote patient monitoring to patients with chronic conditions

  • Collaborate with the healthcare team to ensure seamless patient care and minimize hospitalizations

  • Participate in regular team meetings and contribute to quality improvement initiatives

Requirements:

  • Active NP or PA license in the state of practice

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills

  • Masters' degree in Nursing or Physician Assistant Studies

Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting.
