About the Role:

We're seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. As a key member of our clinical team, you'll build strong relationships with the facility, maintain a patient-centered approach, and be a collaborative team player.

Responsibilities:

Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home
Conduct patient assessments, develop treatment plans, and provide education to patients and families
Collaborate with facility staff to ensure seamless care transitions and comprehensive patient care
Monitor patient progress, identify trends, and adjust care plans accordingly
Maintain accurate and timely documentation, supported by a dedicated scribe team

Requirements:

Active NP or PA license (in the state of practice)
1+ year of experience in:
- Geriatric care
- Documentation review
Excellent communication, collaboration, and teamwork skills
