As a member of the clinical team, the Nurse Practitioner will bring an advanced knowledge and diverse skill-set to assess the healthcare of chronically ill geriatric members, in collaboration with physicians and interdisciplinary teams. Additionally, candidates must be willing to participate in continuing education/workshops to maintain his/her valid license. A minimum of one year’s experience working with an elderly community is required, as is a strong passion to help this unique population each day. EOE
