Responsibilities

As a Hospice CNA You will:   

  • Personal Care: Help with bathing, dressing, toileting, mobility, and repositioning 
  • Monitor & Report: Observe and report changes in the patient’s condition 
  • Mobility: Assist with ambulation, transfers, ROM exercises and safe equipment use 
  • Medication Assistance: Help with self-administered medications per state regulations  
  • Emotional Support: Offer companionship and comfort to patients and families. 
  • Light Housekeeping: Change linens, assist with meals, and maintain a clean space. 
  • Documentation: Maintain accurate and timely documentation   
  • Team Participation: Collaborate with hospice interdisciplinary team   

Qualifications

  • Meet the training requirements of the state of practice
  • Minimum of six months experience including providing personal care
  • Must be 18+ years of age
  • Complete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical training
  • Possess and maintain current CPR certification
  • Ability to read and follow written instructions and document care given
  • Understands hospice philosophy, comfortable providing specialized care to the terminally ill
  • Ability to work with little direct supervision
  • Ability to handle death/dying
  • Strong oral and written communication skills
  • Good Organizational Skills

Under direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care and perform bladder scans. Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation. Coordinates tests and treatments with and between various departments for patients on nursing units. Maintains a working knowledge of functions and services offered by all diagnostic and treatment departments to facilitate the scheduling of tests and treatments. Maintains working knowledge of EHR system and other computer systems for ordering patient and unit supplies and equipment. Processes daily patient charges and/or related procedures through the system. Facilitates communications required to support patient care, including the multiple-line telephone, unit/site paging and the hospital information system. Screens incoming telephone calls. Monitors department office equipment and troubleshoots minor problems. Arranges repair of major malfunctions. Compiles necessary chart forms for admissions, transfers, discharges and expirations. Obtains appropriate copies of medical records for transfers to nursing homes and other health care facilities.
