We are seeking a highly skilled and compassionate MDS Coordinator to join our team at Winchester Rehabilitation and Nursing Center, a 120-bed skilled nursing facility located in Winchester, MA. As a valued member of our team, you will play a crucial role in supporting high-quality, resident-centered care through comprehensive assessments, care planning, and coordination with our interdisciplinary team.

About the Role:

As a MDS Coordinator, you will be responsible for conducting and coordinating the development and completion of resident assessments, implementing resident Care Area Assessments (CAAs), and working collaboratively with the interdisciplinary care team to develop comprehensive assessments and individualized care plans for each resident. You will also participate in facility survey readiness activities and support the facility throughout the survey process.

Responsibilities:

• Conduct and coordinate the development and completion of resident assessments
• Conduct resident assessments in accordance with current rules, regulations, and guidelines
• Implement resident Care Area Assessments (CAAs)
• Work collaboratively with the interdisciplinary care team to develop comprehensive assessments and individualized care plans for each resident
• Participate in facility survey readiness activities and support the facility throughout the survey process
• Ensure assessment and care planning processes are completed accurately and in accordance with applicable requirements
• Collaborate with nursing staff and other healthcare professionals to support quality resident outcomes

Qualifications:

• Current, unencumbered Massachusetts RN or LPN license
• Minimum of two years of MDS and PPS experience
• Previous care planning experience
• Strong organizational, communication, and assessment skills
• Ability to work effectively with an interdisciplinary healthcare team
• Commitment to providing high-quality, resident-centered care
