Schedule:

Every Monday: 7:00 PM – 7:00AM
Every other Sunday: 7:00 PM – 7:00 AM

Your Impact as a Memory Care LPN:

As an LPN, you will play a critical role in ensuring residents receive safe, compassionate, and high-quality care. Your responsibilities will include:

Conducting resident assessments and supporting individualized care plans
Administering medications and treatments accurately
Monitoring resident conditions and communicating changes appropriately
Ensuring proper documentation of care and services
Supporting and guiding caregiving staff during overnight shifts
Promoting a calm, safe, and supportive environment for memory care residents
Helping residents maintain dignity, comfort, and quality of life

Core Responsibilities:

Medication administration and treatment management
Resident monitoring and clinical support during NOC hours
Accurate and timely documentation
Collaboration with care team and leadership
Adherence to all state regulations and clinical standards

What We're Looking For:

Active LPN license in Illinois
Experience in memory care, assisted living, or long-term care preferred
Strong clinical judgment and attention to detail
Ability to work independently during overnight shifts
Compassionate, patient, and team-oriented approach
Reliable and committed to consistent scheduling
