Caregiver Responsibilities:

Our caregivers provide compassionate hands-on support to clients by assisting them with daily activities such as personal care, meal preparation, cleaning, mobility, medication reminders, transportation, and companionship.


Caregiver Requirements:

  • 18 years of age or older

  • HS Diploma/GED Equivalent

  • Authorized to work in the US

  • Driver's license with vehicle

  • 1 year of caregiving experience preferred

  • Lift/transfer experience a plus!

  • Ability to pass a background check

  • Ability to lift at least 50 lbs

Experienced Caregiver Needed for Franklin-Area Client Location: Franklin, Tennessee Schedule: Every Monday, Wednesday, and Friday Hours: 12:00 PM–5:30 PM Weekly Hours: Approximately 16.5 hours Position Type: Consistent, part-time home-care assignment Helping Hearts is seeking a reliable, compassionate, experienced, and highly attentive caregiver to support a client in the Franklin area. This client requires full assistance with activities of daily living, including hands-on transfer assistance, and is considered a significant fall risk. We need a caregiver who understands that fall prevention requires constant awareness, close supervision, careful assistance, and proactive safety measures throughout the entire shift. The selected caregiver must be available to work all three scheduled days Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Consistent attendance is essential because the client depends on a stable routine and familiar caregivers. The client also has a small dog in the home. Applicants must be comfortable entering and working in a home with a small household pet. At Helping Hearts, caregiving is more than a job, it is a calling. We are looking for someone who will treat the client with dignity, patience, compassion, and respect while remaining actively engaged and focused on the client’s safety. Primary Responsibilities Full Assistance with Activities of Daily Living The caregiver will provide respectful, hands-on assistance with the client’s daily personal-care needs as outlined in the client’s care plan. Responsibilities may include: Bathing or personal-hygiene assistance Dressing and undressing Grooming and oral-care support Toileting and incontinence care Mobility assistance Positioning and repositioning Safe transfer assistance Wheelchair and mobility-device assistance Meal preparation and hydration reminders General comfort and personal-care support Maintaining the client’s privacy and dignity during all care tasks Care must be provided patiently and professionally without rushing the client. The caregiver must follow the established care plan and use only approved transfer techniques and equipment for which the caregiver has received proper training. Safe Transfer Ability Is Required This client requires hands-on assistance with transfers. Applicants must have the physical ability and willingness to safely assist the client with transfers, including: Wheelchair to commode and commode to wheelchair Wheelchair to bed and bed to wheelchair Wheelchair to chair or other approved seating Sit-to-stand and stand-to-sit transfers Toileting-related mobility and clothing-management assistance Repositioning for safety, hygiene, and comfort Transfers using approved equipment when required by the care plan Previous experience using a Hoyer lift, sit-to-stand lift, or other mechanical transfer equipment is preferred but not required. Applicants without this experience must be willing and able to complete all required hands-on training and demonstrate safe use of the equipment before independently performing mechanical-lift transfers. During every transfer, the caregiver must: Explain the transfer process to the client Make sure the client is positioned correctly Properly position the wheelchair or commode Lock the wheelchair brakes before beginning Move footrests, cords, furniture, and other obstacles out of the way Clear and inspect the transfer area Use proper body mechanics Use approved transfer equipment when required Closely monitor the client’s balance and comfort Move slowly and avoid rushing or pulling the client Stop immediately if the transfer becomes unsafe Transfers must never be rushed or attempted in an unsafe manner. The caregiver must follow the client’s care plan and use any required gait belt, Hoyer lift, sit-to-stand lift, or other approved transfer device only after receiving proper training and authorization. If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction. Applicants must be able to perform the essential transfer responsibilities of this position safely and consistently, with or without reasonable accommodation. Proactive Fall Prevention and Safety Monitoring Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety-focused throughout the entire shift. This is not a passive companion assignment or a position where the caregiver can sit nearby without actively observing and assisting the client. The caregiver will be expected to: Remain aware of the client’s location, mobility, and immediate needs Provide close supervision during standing, walking, toileting, and transfers Stay within a safe assisting distance whenever the client is moving Encourage the client to request assistance before attempting to stand or transfer Maintain a clear and hazard-free environment Check for spills, cords, loose rugs, clutter, poor lighting, and unsafe furniture placement Make sure mobility devices are properly positioned and within reach Make sure the client has appropriate footwear during mobility Keep commonly used items safely within the client’s reach Remain especially attentive when the client is tired, weak, distracted, or attempting to toilet Never leave the client unattended during a task when supervision is required Avoid personal-phone use or other distractions that interfere with active supervision Immediately report changes in mobility, balance, strength, or transfer ability Immediately report every fall, near fall, injury, or safety concern Complete all required incident documentation accurately and promptly The caregiver must never attempt an unsafe lift or transfer. If the client cannot be moved safely according to the care plan, the caregiver must protect the client from further injury and contact the Helping Hearts office immediately for direction. Active Engagement and Companionship The client needs a caregiver who will be present, attentive, conversational, and genuinely engaged. Meaningful companionship is an important part of this assignment. The caregiver should: Initiate friendly and appropriate conversations Offer reassurance, encouragement, and emotional support Participate in activities the client enjoys Help maintain a calm, positive, and familiar daily routine Recognize when the client may be lonely, anxious, uncomfortable, or withdrawn Encourage safe participation in daily activities whenever appropriate Respect the client’s choices, preferences, personal space, and independence Remain patient if the client needs additional time, repetition, or reassurance Provide companionship without becoming distracted by a personal phone, television, or unrelated activities We are looking for someone who understands that quality caregiving includes both physical assistance and meaningful human connection. Meal Preparation, Snacks, and Hydration Depending on the client’s care plan and preferences, responsibilities may include: Preparing simple meals and snacks Encouraging appropriate food and fluid intake Providing hydration reminders Assisting with meals when required Cleaning food-preparation areas after use Washing dishes and returning items to their proper locations Observing and reporting appetite changes, swallowing concerns, nausea, or refusal of food or fluids Following all dietary instructions listed in the client’s care plan Attention to cleanliness, nutrition, hydration, and the client’s comfort is essential. Light Housekeeping and Home Support The caregiver will help maintain a clean, organized, comfortable, and hazard-free living environment. Responsibilities may include: Tidying commonly used areas Washing and putting away dishes Completing light laundry Changing bed linens when needed Wiping down frequently used surfaces Keeping the bathroom and personal-care areas clean Removing trash Returning supplies and mobility equipment to their proper locations Keeping floors and walking paths free from fall hazards Reporting low supplies, maintenance concerns, or household safety hazards The client’s direct care, supervision, and safety must always remain the caregiver’s first priority. Observe, Document, and Report Changes The caregiver must closely observe the client’s overall condition and promptly report concerns to Helping Hearts. This includes changes involving: Mobility, balance, or transfer ability Physical strength or coordination Skin condition, bruising, redness, swelling, or pain Appetite or fluid intake Toileting patterns Mood, behavior, or cognition Alertness, confusion, agitation, or unusual behavior Breathing, comfort, or general condition The safety or cleanliness of the home Supplies that are low or unavailable Any fall, near fall, injury, or other incident Caregivers must complete accurate shift documentation, electronic visit verification, and required care notes through the company’s approved system. Concerns must be reported immediately. Caregivers must not wait until the end of the shift to report an urgent change, fall, near fall, injury, or safety concern. Small Dog in the Home The client has a small dog. Applicants must: Be comfortable entering and working in a home with a small dog Remain aware of the dog’s location when the client is walking or transferring Help prevent the dog, toys, food bowls, or leashes from becoming fall hazards Follow the client’s instructions regarding appropriate interaction with the dog Never allow the pet to interfere with the client’s care, supervision, or safety Routine pet care is not automatically part of the assignment unless it is specifically included in the client’s approved care plan. WellSky Documentation Complete all required documentation accurately and timely through WellSky Personal Care . Documentation must clearly reflect care provided, observations, client status, activities completed, and relevant concerns. Communication With Operations Maintain clear, timely, and professional communication with Helping Hearts Operations. When an update is requested, caregivers must provide a meaningful written response rather than a one word answer. Professional Boundaries Maintain appropriate boundaries with the client and family. Scheduling, pay, additional hours, time off, and employment related matters must be handled through Helping Hearts Operations. Shift Handoff Provide an appropriate handoff to an incoming caregiver when applicable, including relevant observations, safety concerns, changes in routine, and unfinished care needs. Full Shift Commitment Remain for the complete scheduled 7:00 AM to 7:00 PM shift unless released by Helping Hearts Operations. Emergency Response Remain calm, protect the client from additional harm, contact emergency services when appropriate, follow Helping Hearts emergency procedures, and notify Operations promptly. REQUIRED SKILLS The strongest candidate will have demonstrated experience with personal care, bathing assistance, toileting support, safe mobility assistance, fall prevention, client supervision, meal preparation, hydration encouragement, companionship, recognizing changes from baseline, professional documentation, and communicating concerns appropriately. We also need someone with strong situational awareness and judgment . You should know when to provide assistance, when to encourage independence, when something does not appear normal, and when it is time to immediately contact Operations. THE PERSONALITY WE NEED Clinical skills and caregiving experience matter, but personality matters for this assignment too. We are looking for someone who is upbeat without being overwhelming, confident without being controlling, compassionate while maintaining professional boundaries, observant without being intrusive, patient, conversational, encouraging, and naturally engaging . The ideal caregiver does not need to be reminded repeatedly to interact with the client or find appropriate ways to keep the day positive and productive. We need someone who walks into the assignment ready to make a positive difference. RELIABILITY IS ESSENTIAL Continuity of care is extremely important. The client, family, and agency need to be able to depend on the selected caregiver every: Wednesday from 7:00 AM to 7:00 PM Friday from 7:00 AM to 7:00 PM Sunday from 7:00 AM to 7:00 PM Applicants with repeated availability conflicts, frequent call outs, ongoing schedule change requests, or an inability to consistently arrive for a 7:00 AM start will not be the right fit for this assignment. AWAKE AND ENGAGED SHIFT This is an awake, active, and engaged caregiving assignment . Sleeping while on shift is strictly prohibited. The caregiver must remain awake, alert, responsive, attentive, and available to the client throughout the entire scheduled shift. Personal phone use must remain limited and may never interfere with client care, safety, supervision, companionship, communication, or documentation. PROFESSIONAL APPEARANCE Helping Hearts caregivers represent our agency every time they enter a client’s environment. Navy scrubs are required. Professional closed toe footwear is required. Crocs are not permitted. Caregivers are expected to maintain a clean, neat, and professional appearance throughout the assignment. QUALIFICATIONS Previous professional caregiving experience is strongly preferred, particularly experience providing one on one care to older adults or clients requiring mobility and fall prevention support. Experience as a Caregiver, CNA, Home Health Aide, Personal Care Assistant, Direct Support Professional, or similar direct care professional is preferred. Applicants must have dependable transportation and be able to consistently arrive on time for a 7:00 AM start . Candidates must be comfortable using a smartphone for electronic documentation and agency communication. Applicants must successfully complete Helping Hearts’ required onboarding process, applicable criminal background screening, registry checks, and required training. Current CPR and First Aid certification is strongly preferred .
