The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

 

The Clinic Manager, LPN, under the supervision of the Regional Director, manages the flow of the clinic, leading all staff in provision of medical and nursing care, ensuring high quality standards and regulatory compliance.  Coordinates services with primary care team and acts as a primary liaison for the clinic and nursing staff.  Triages participants as needed and is the gatekeeper for daily clinic scheduling.  Provides basic care to participants; implements physician, physician assistant and nurse practitioner orders.  Provides training and education to participant, caregiver family and staff; and communicates participant’s condition to caregivers, primary care physician, and the interdisciplinary team.  Has direct oversight of all responsibilities for the staff in the clinic, (excluding providers) including clinical educations, training/observations, reporting requirements, and competency evaluation.

 


Qualifications

Clinic Manager, LPN qualifications include:

  • A Licensed Practical Nurse, must have graduated from an accredited school of nursing and possesses a current license, in good standing, from the Commonwealth of Pennsylvania.
  • 2 to 3 years of nursing experience, including 1 year supervisory experience preferred, with 1 year experience working with a frail or elderly population preferred.
  • Working knowledge of computers, Microsoft Word and Excel required.

Position Summary The Licensed Practical Nurse (LPN) is responsible for providing direct nursing services within an outpatient addiction treatment setting. The LPN works closely with physicians, nurse practitioners, registered nurses, counselors, and other treatment professionals to support client health, safety, and recovery outcomes. Responsibilities include medication administration, nursing assessments, withdrawal monitoring, patient education, and clinical documentation. Essential Responsibilities Provide direct nursing care and clinical support to clients. Administer medications and monitor effectiveness and side effects. Complete nursing assessments, screenings, and documentation. Monitor withdrawal symptoms using tools such as COWS and CIWA. Educate clients on medications, health conditions, and recovery-related topics. Support providers during medical evaluations and medication management appointments. Communicate significant health concerns and changes in condition to the treatment team. Maintain medication storage, clinical supplies, and medical equipment. Collaborate with interdisciplinary team members to ensure continuity of care. Maintain confidentiality and adhere to all professional, regulatory, and organizational standards. Qualifications Required Current, active, and unrestricted Pennsylvania LPN license Current CPR/BLS & First Aid certification Possession of strong communication and organizational skills Ability to work independently and as part of a multidisciplinary team Preferred Experience in addiction medicine, substance use disorder treatment, or behavioral health Knowledge of withdrawal management protocols and medication-assisted treatment (MAT) Experience working with individuals experiencing co-occurring mental health and substance use disorders.
