Hospice Clinical Manager (RN)

Job Summary:

As a Hospice Clinical Manager (RN) with LiveWell Partners, you will play a vital role in leading our team of compassionate healthcare professionals in providing exceptional care to patients in their homes. As a clinical leader, you will oversee the day-to-day operations of our hospice services, ensuring that our patients receive holistic care that addresses their physical, emotional, and spiritual needs.

Responsibilities:

Oversee the coordination of hospice care services, ensuring that patients receive comprehensive care that meets their unique needs
Develop and implement effective care plans that promote symptom management, comfort, and quality of life
Provide clinical guidance and oversight to a team of interdisciplinary healthcare professionals, including nurses, social workers, and other clinicians
Foster strong relationships with patients, families, and healthcare providers to ensure seamless continuity of care
Conduct ongoing assessments and evaluations to ensure that patients are receiving high-quality care that meets industry standards
Collaborate with the hospice care team to address patient and family concerns, and develop strategies for improving patient satisfaction and outcomes
Develop and maintain relationships with community partners, healthcare providers, and payors to ensure access to resources and services for our patients
Participate in quality improvement initiatives to promote continuous learning and improvement in hospice care

Requirements:

Current RN license in the state of Michigan
Minimum of [insert number of years] years of experience in hospice care or a related field
Strong clinical expertise in hospice care, including symptom management, pain and palliative care, and end-of-life care
