About the Role: Licensed Practical Nurse

As a LPN, you’ll play a key role in supporting the health, happiness, and daily experience of the residents at The Village in Indianola. 

As a LPN, you will:  

  • Conduct comprehensive assessments of residents' health conditions, including vital signs, and develop individualized care plans. 
  • Administer medications and treatments as prescribed, ensuring accurate documentation and monitoring for side effects. 
  • Provide direct nursing care, such as wound care, IV therapy, and catheter management. 
  • Collaborate with the healthcare team to coordinate and optimize resident care, including communicating with physicians and families. 
  • Supervise and support CNAs and other healthcare staff, providing guidance and training as needed. 
  • Ensure compliance with healthcare regulations and infection control protocols to maintain a safe environment for residents. 
  • Offer emotional support and education to residents and their families, promoting overall well-being and understanding of care plans. 

Open Shifts Available: 

  • 1st shift, Part-time, 12 hour shifts Rotating Weekends
  • 2nd shift, Full-time, 8 hour shifts Rotating Weekends
  • PRN

Qualifications: 

  • Compassionate, dependable, and motivated to serve others

  • State of Iowa LPN License

Multi-Site MDS Coordinator - RN/LPN

Key Responsibilities Complete, review, and submit MDS assessments, CAAs, and comprehensive care plans in accordance with CMS RAI guidelines. Manage MDS schedules, ARDs, Medicare assessments, IPA reviews, and Medicaid Case Mix processes. Ensure accurate clinical documentation, ICD coding, and regulatory reporting. Monitor MDS quality measures, validation reports, and compliance metrics; complete corrections as needed. Educate and support nursing and interdisciplinary team members on MDS best practices and documentation standards. Participate in Medicare meetings, Triple Check processes, QAPI initiatives, and quality improvement activities. Identify significant changes in resident condition and coordinate appropriate assessments. Travel to assigned communities to provide onsite support, training, and coverage as needed. Qualifications Graduate of an accredited nursing program. Current Iowa RN or LPN license in good standing. Minimum 2 years of long-term care experience . Minimum 2 years of dedicated MDS experience . RAC-CT certification required within one year of hire and maintained thereafter. Strong knowledge of CMS RAI guidelines, Medicare, Medicaid, and long-term care regulations.
