Responsibilities:

  • Assisting patients with activities designed to maintain personal hygiene such as bathing, grooming, and skin care.

  • Assisting patients with ambulation, transfers, and turns.

  • Ensuring a clean and orderly patient environment to promote safety and infection prevention.

  • Notifying clinical staff of any changes in a patient’s condition or safety concerns.

  • Applying infection control concepts and techniques.

  • Monitoring, recording, and effectively communicating the patient’s condition, activities, behaviors, and safety concerns utilizing computerized documentation systems and other means as appropriate.

  • Taking vital signs and performing phlebotomy (blood draws) and glucose checks.

Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department.
