The Registered Nurse's primary function is guiding breast health patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.
Job Responsibilities include:
Participate in multidisciplinary clinic, with providers and patients, for discussion of individualized breast treatment options.
Prepare for and support the patient and health care team during procedures.
Guide patients through the care system.
Connect patients to community and social support services.
Offer educational sessions to inform practitioners of breast health patient navigator role, services provided.
Provide breast health education to individuals and groups.
Assist with arranging for patient transportation when needed.
Job Requirements:
The ideal candidate will be an RN with a background and familiarity with breast health, knowledgeable in regard to computers and strong
community resources and hospital processes. Excellent customer service skills a must.
