Position Description:

  • The Nursing Supervisor will be responsible for the on-site supervision and assist with management of Nursing services for each branch. They will report directly to the Branch Director and partner with the clinical and quality team to support broader initiatives
  • The RN Field Staff will indirectly report to the RN Supervisor with overall direction coming from Branch Director and support from Clinical, Quality and Education.
  • The RN Supervisor Role is a hybrid per- visit clinical role with reduced productivity expectations to allow additional administrative time to attend to branch required responsibilities and duties. (Allocated hours will vary week to week and branch to branch based on needs and approval by Branch Director)
  • Meet with Branch Director weekly to review and plan each week’s agenda and duties (Interviewing, hiring, onboarding, on-site skills assessments and supervisory visits.)
  • Train LVN RN (If applicable) to perform basic in-services for Business Development team.
  • Assist Branch Director and Rehab Management with staffing including weekend and holiday Rehab coverage schedules.
  • Act as spokesperson during Care Team Conference to share any updates, align on key metrics, discuss trends or regulatory issues.
  • Oversee and run monthly nursing meeting for their branch. Agenda to be provided by Nursing Cohort and Branch Director.
  • Will assist in regulatory oversight of all quality initiatives including
  • Will assist in (POC) action plans, developed post site survey credentialing and complaint visits.

    • Guide the new employee in the orientation process by teaching the roles and responsibilities of the job as well as standards of behavior, formal and informal rules, and culture.  Assists in completion of orientation checklists and competency validation.
    • Oversee the learning experience for students.
    • Mandatory attendance on new hire update calls.
    • Assist existing employees with roles and responsibilities of the job such as standards of behavior, formal and informal rules, and culture. Assists in education on new and existing processes and programs. Assisting with annual skills competencies.
    • Available for clinical conferencing and consulting from team members.
    • Attend mandatory quarterly team meetings with the education department and fellow preceptors for updates and discussion.

Qualifications:

  • Graduate of an accredited school of nursing. 2+ years of recent acute care experience in an institutional setting. Bachelor’s degree preferred.
  • 1 year of Home Health experience required. 3+ years of Home Health experience preferred.
  • Have valid driver’s license with no restrictions and auto insurance meeting state’s insurance coverage minimums.
  • Current licensure in State, CPR certification.
  • Supervisory experience not required but preferred.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Demonstrates an enthusiasm for the nursing and therapy profession and projects a positive attitude.
  • Conveys a commitment to personal and professional growth.
  • Consistently adheres to the organization’s policies, procedures, and protocols.
  • Annual performance evaluation is within standards and has received no counseling for policy or conduct violations.

Pre-Admit Staff Nurse - Per Diem

Bargaining Unit: EAP Rate of Pay: $79.54/hour + DOE (Base Pay of $70.71/hour includes 12.5% per diem differential) Summary Utilizes the nursing process to coordinate and plan quality care for surgical clients of all age groups undergoing surgical procedures. Coordinates the pre-admission process in collaboration with Physician’s offices, Patient Registration Department, Anesthesia, and Surgical Services Departments. Coordinates healthcare services for patients in need of additional education or resources for the management of their disease in advance of an impending procedure. Coordinates educational resources, provider orders and ensures proper testing, labs, imaging, and medical records are completed and results are available prior to scheduled procedures. Assists patients through the healthcare system and coordinates peri-operative needs with all internal and external stakeholders, including primary and specialty care teams, operating room staff, anesthesiologists, hospitalists, and the patient/ patient's representatives. Works directly with peri-operative provider team to achieve a standardized, best practice and outcomes-based approach to patients and their perioperative needs, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting. Coordinates continuity of patient care with internal and external healthcare providers, including the patient's primary care physician. Coordinates continuity of patient care with patients, their family, and primary care physician following hospital admission, discharge, and follow up calls. Facilitates patient medication management under the guidance of Anesthesiologist, Pre-Surgery Physician, or the patient's primary care team. Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans. Utilizes equipment properly.
