About the Role

As a Nurse Practitioner at Carease Health, you will play a vital role in our comprehensive preventative care program, which integrates Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM). You will focus on proactive measures to enhance physical health and well-being, while also taking a proactive approach to mental health. This strategic integration will minimize hospitalizations, elevate patient care standards, and increase reimbursement for your facility.

Key Responsibilities

  • Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work

  • Demonstrate a patient-centered approach, building strong relationships with residents and facility staff

  • Collaborate with our clinical team to ensure seamless alignment with facility processes and protocols

  • Conduct thorough assessments, diagnoses, and treatment plans for patients

  • Utilize Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) to monitor vital signs and detect abnormalities

  • Develop and implement individualized care plans to promote physical and mental well-being

  • Participate in quality improvement initiatives to minimize hospitalizations and improve patient outcomes

Requirements

  • Active NP or PA license in the state of practice

  • 1+ year of experience in Geriatric care and Documentation review

  • Excellent communication, collaboration, and teamwork skills
