As a Nurse Practitioner at Carease Health - WI, you will be part of a hybrid role that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

  • Provide high-quality preventative care to residents in skilled nursing facilities

  • Conduct on-site visits and remote work from home, supported by a dedicated scribe team

  • Build strong relationships with facility staff and maintain a patient-centered approach

  • Collaborate with other healthcare professionals to ensure seamless patient care

  • Maintain accurate and timely documentation of patient visits and treatment plans

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills

  • Masters-level education in Nursing or a related field
