We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
Manage Residents with acute change in condition.
Advanced care planning and chronic care management.
Prescribe appropriate medications, including controlled substances in accordance with Colorado Law.
Order, review and/or follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
and/or the care plan.
Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

Current, unrestricted NP license in the state of Colorado.
Board Certification.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience working in a long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team to coordinate patient care.
Psych experience a plus

Overview NaphCare is hiring a MAT Nurse Practitioner to join team of medical professionals in the Pueblo County Jail in Pueblo, CO . As a MAT Nurse Practitioner , you will work alongside correctional officers and other medical professionals to provide support to the patients within the facility struggling with substance abuse. Responsibilities The MAT Nurse Practitioner plays a critical role in our continued mission to provide quality care that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and are not limited to: Provide comprehensive Medication-Assisted Treatment (MAT) services for incarcerated individuals with Opioid Use Disorder (OUD), with a primary focus on buprenorphine/naloxone (Suboxone). Conduct initial and ongoing clinical assessments for OUD, including substance use history, withdrawal evaluation, psychiatric and medical comorbidities, and readiness for treatment. Initiate, continue, adjust, and monitor Suboxone therapy in accordance with evidence-based guidelines, DEA regulations, and correctional health policies. Manage induction, stabilization, maintenance, and tapering phases of MAT as clinically indicated. Collaborate closely with correctional healthcare staff, custody staff, mental health providers, and substance use counselors to ensure coordinated, safe, and effective care. Monitor patient adherence, response to treatment, and potential diversion risks within a correctional setting. Provide patient education on OUD, MAT, relapse prevention, overdose risk, and continuity of care upon release. Participate collaboratively with an interdisciplinary team to create individualized treatment plans that address medical, behavioral health, and social determinants of health. Ensure accurate, timely, and compliant documentation in the electronic health record, including MAT-specific reporting and regulatory requirements.
