NP ROLE AND RESPONSIBILITIES: Provide primary and preventative care for long term care patients residing in skilled nursing facilities. Manage Residents with acute change in condition. Advanced care planning and chronic care management. Prescribe appropriate medications, including controlled substances in accordance with Iowa Law. Order, review and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with primary care physician. NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Iowa. Board Certification. DEA licensure. Clinical background in adult, family or geriatric specialties. Experience working in a long-term care setting. Ability to work with an EMR. Ability to manage patients with advanced illness and multiple chronic conditions. Ability to work within an interdisciplinary care team to coordinate patient care. How you'll contribute The Education Coordinator is the organization-wide educator. By maintaining competency and knowledge of current standards of practice, trends, and developments in requirements for acute care hospital and all applicable facility-wide healthcare settings, the Education Coordinator designs, develops, plans, and teaches educational programs for the employed, contracted, affiliated, volunteer individuals and others as identified. The educator has the authority and responsibility of supervision all orientation programs and has indirect supervisory responsibility for new employees when involved in educational programs or processes. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins. Associate's degree from an accredited professional Registered Nursing Program Basic Life Support (BLS) Prior experience as a nurse educator preferred.
