This is a great opportunity for a nurse who understands clinical care and wants to use that knowledge in a role focused on medical records, regulatory compliance, clinical documentation, and interdisciplinary coordination.

As our Medical Records LPN, you'll play an important role in making sure our residents' clinical records are complete, accurate, confidential, and survey-ready while working closely with nursing, providers, and other members of our interdisciplinary team.

What You'll Do

Clinical Records & Documentation

• Maintain complete, accurate, current, and compliant clinical records for residents.

• Ensure documentation is appropriately dated, signed, legible, and securely maintained or scanned.

• Manage admission, transfer, discharge, and death records and required logs.

• Maintain the master patient census and resident master index.

• Help ensure clinical documentation remains organized, accessible, and survey-ready.

Auditing & Compliance

• Conduct routine audits of clinical records, physician orders, MAR/TAR documentation, charting, and assessments.

• Monitor required admission, quarterly, annual, discharge, and physician documentation.

• Track physician visits, history and physical examinations, signed orders, and other required documentation.

• Identify documentation deficiencies and work collaboratively with the interdisciplinary team to resolve them.

• Help support ongoing regulatory and HIPAA compliance.

Admissions, Discharges & Transitions

• Review clinical documentation for new admissions and readmissions.

• Support admission, discharge, and transfer documentation and workflows.

• Assist with new admission and readmission orders.

• Coordinate with nursing and the interdisciplinary team to help ensure smooth transitions and complete records.

Release of Information & Reporting

• Process medical record and Release of Information requests while maintaining resident confidentiality.

• Gather documentation requested by auditors, insurance carriers, Medicare, Medicaid, VA, and regulatory agencies.

• Complete required monthly and regulatory reports, including death reports and Ombudsman discharge reporting.

Records Management

• Maintain secure systems for active and inactive clinical records.

• Track records maintained in off-site storage.

• Ensure records are retained, protected, and accessible in accordance with applicable requirements.

• Order and maintain clinical record forms and related supplies.

Collaboration & Operational Support

• Partner with nurses, providers, leadership, and other departments to improve documentation accuracy and timeliness.

• Communicate audit findings and follow-up needs with the interdisciplinary team.

• Participate in care conferences, resident engagement rounds, and Manager of the Day responsibilities as assigned.

What We're Looking For

• Current Oregon LPN license in good standing with the Oregon State Board of Nursing

• At least 2 years of experience in nursing, healthcare administration, medical records management, or a related healthcare setting

• Strong attention to detail and organizational skills

• Ability to manage multiple priorities and deadlines

• Strong written and verbal communication skills

• Knowledge of HIPAA and healthcare regulatory requirements

• Familiarity with medical terminology, including ICD-10-CM coding and indexing

• Experience working with electronic medical record (EMR/EHR) systems

• RHIT experience or credential preferred

• A collaborative, dependable approach to working with an interdisciplinary healthcare team
