Partner with Community Health Network and Deaconess Health System – IHCI
The Innovative Healthcare Collaborative of Indiana (IHCI) is a joint venture between Community Health Network and Deaconess Health System. Its goal is to support our sponsors and partners in their strategic evolution to positively impact and improve the healthcare delivery system.
Make a Difference
As a Discharge Care Manager LPN on the IHCI Team, you will have the opportunity to have a profound impact on the lives of people living with complex and/ or chronic conditions. The Discharge Care Manager LPN connects with the patients on the phone, or in person, in the physician offices, or in the hospital setting. The IHCI LPN Post-Discharge Care Manager will be providing telephonic with some face-to-face interaction with patients and their care team as appropriate to improve patient care. Along with other members of the Population Health team, conduct patient surveys that include the medical, behavioral, pharmaceutical and social needs of the patient, identify gaps in care and barriers to attaining improved health. Based on this patient survey, and in conjunction with the patient, the patient’s physician and other members of the population health team manage a care plan that will address the identified needs, remove the barriers and improve the health of the patient. Coordinate care by serving as the contact point, advocate and resource for the patient, their family and their physician, building effective relationships through trust, respect and communication. In close collaboration with the patient, primary care provider and care team you will continually monitor the patient’s knowledge of their clinical condition(s) and provide education and self-management support based on the patient’s unique learning style. The Discharge Care Manager LPN will measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.

Exceptional Skills And Qualifications
- 2 year / associate degree in practical nursing. (required)
- Licensed as a Licensed Practical Nurse (LPN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC). May require Illinois and/or Kentucky licensure in the future. (required)
- 3+ years: Experience in home health, ambulatory care, community public health, case management, or coordinating care across multiple settings and providers. (required)
- Population Health management experience. (preferred)
